HOW DO CHEEKS AGE?
The base-up triangle changes to a base-down triangle as we age, and, even worse, may become a rectangle. A rectangular face looks better on men than on women. Besides the jawline and jowls, the cheek volume loss, flattening and inferior movement of the cheek fat pads have a lot to do with this aging. It is impossible to obtaine a beautiful base-up triangle just with a face and neck lift if the prominence of the cheeks, the volume and position of the cheek eminence are not addressed.
AGING OF CHEEKS SUPERIMPOSED ON MORE GENERALIZED AGING
Even though there is generalized aging of all parts of the face and of all the layers, the midfacial changes are especially impressive with volume loss, loss of prominence of the cheeks, loss of the sub-malar hollow (the model’s hollow is what I call it), the smooth “apple-cheek” roundness, the smooth nasolabial fold, the clean transition from the lower eyelids to the cheeks and the healthy appearance of the skin.

HOW DO THE CHEEKS AGE?
Traditionally, plastic surgery has concentrated on the aging changes that affect the brows, upper eyelids, lower eyelids and the lower face and neck. The forgotten cousin in all this has been the midface. One only has to look at the shape of the midface in youth to appreciate the important contribution of the cheek fullness and angle to beauty. In men as well as women, creating the perfect cheek prominence (different in the two sexes, of course) allows one to design a pleasing and youthful face. Ignoring the cheek, as it is often done in cosmetic surgery, leads to an incongruous result. In the “old days”, one use cheek implants exclusively. Whereas there is definitely a place for cheek implants, advances in our understanding of aging of the midface, anatomical changes that occur and small incision surgery now allows us to use an amalgam of procedures to give a natural and un-operated look.
